From North (Boston)

Route 128 to Exit 4, Route 24 South to Exit 15, Route 104 East, follow 3 miles.  Take left at Stop Sign to Bridgewater Rotary.  Take first right out of Rotary onto Routes 18/28 South.  Conant Community Health Center is one mile south on Wally Krueger Way on right side of road. First building on Wally Krueger Way is the Conant Center.

From South (Fall River)

Route 24 North to Exit 15, Route 104 East, follow 3 miles.  Take left at Stop Sign to Bridgewater Rotary.  Take first right out of Rotary onto Routes 18/28 South.  Conant Community Health Center is one mile south on Wally Krueger Way on right side of road. First building on Wally Krueger Way is the Conant Center.

From East (Cape Cod)

Route 495 North to Exit 5, bear right to Rotary. Take Routes 18/28 North.  Conant Community Health Center is 5 miles north on Wally Krueger Way on left side of road.

First building on Wally Krueger Way is the Conant Center.

From West (Worcester)

Route 495 South to Exit 7A, Route 24 North.  Route 24 North to Exit 15, Route 104 East, follow 3 miles.  Take left at Stop Sign to Bridgewater Rotary.  Take first right out of Rotary onto Routes 18/28 South.  Conant Community Health Center is one mile south on Wally Krueger Way on right side of road.  First building on Wally Krueger Way is the Conant Center.

Wally Krueger Way is the little street located just between a pet grooming shop and Signature Healthcare on Routes 18/28. The Bridgewater Senior Center is also located on Wally Krueger Way. The Conant Health Center is the first building you come to as you turn on to Wally Krueger Way from Routes 18/28.
